JOB DESCRIPTION:
'A2; Care Coordinators assist patients with managing their overall health by daily monitoring and
calls. You will have an assigned group of patients to follow up with monthly via phone.
'A2; Conversations include: reviewing patient recorded daily vital readings, educating patients on
improving diagnosed diseases and assisting patients to supporting the plan of care
established by their provider.
'A2; Verify and record the patient’s demographics, medical issues, medications, and risk
management factors to create a clinical summary using specialized medical software.
'A2; Handle potentially difficult conversations calmly and professionally.
'A2; Participate in individual & Group projects as directed by management.
'A2; Strong communication skills – ability to engage patient in conversation, motivate patient to
follow the care plan.
'A2; Make a positive impression with the patients through every interaction, contributing to
positive attitude with team and individual goals.
'A2; Immediate joiners preferred.
