Job Description

  • Collaborate with multidisciplinary clinical teams to enhance patient safety by ensuring safe, appropriate, and cost-effective use of medications, including chemotherapy and immunosuppressive therapies.
  • Conduct audits of patient medical and medication records to identify medication errors, drug interactions, adverse drug events, and associated risks; recommend corrective and preventive strategies.
  • Perform Drug Utilization Reviews (DUR) to evaluate medication usage patterns and ensure rational prescribing.
  • Identify, analyze, prevent, and manage drug-related problems through evidence-based, patient-centric medication therapy recommendations.
  • Participate in interdisciplinary patient care rounds, including transplant units and critical care areas, and communicate therapeutic interventions with the clinical team.
  • Assist the HOD and hospital teams in developing and implementing medication management policies and procedures.
  • Address medication-use process concerns through active participation in relevant hospital committees.
  • Coordinate and participate in monthly Pharmacy & Therapeutics Committee meetings.
  • Participate in interdepartmental meetings and fulfill assigned responsibilities.
  • Manage and support the Antibiotic Stewardship Program.
  • Act as Chapter Champion JCI Medication Management and Use (MMU).
  • Ensure implementation and compliance with all quality and accreditation standards related to medication management.
  • Prepare and maintain departmental MIS as per the defined calendar.
  • Perform additional duties as assigned by the HOD from time to time.
